Terms and Conditions
1) You must be a registered client at one of the Bayfields Opticians and Audiologist practices to be eligible for the Belong plan.
2) Offer is available to those who set up monthly direct debit payments (one yearly payment will not be accepted). It is also available to those clients who were previously on the Bayfields B-Free or B-You payment plans.
3) Where a client is entitled to an NHS funded appointment. The NHS fee will be claimed.
4) If your contact lens specification is not in date we will be unable to supply you with contact lenses, you will need an up to date prescription.
5) Belong clients will be eligible for the entitlements detailed on their plan as soon as they’ve paid their first direct debit.
6) No refunds are available for eyecare, and services already provided.
7) All Belong benefits are redeemable only by the registered person who is signed up to the plan.
8) Membership is valid up until any cancellation of your direct debit payments or failure to collect payment.
9) Belong benefits can be redeemed under the following conditions:
· Free Appointments are available where an appointment is due, where client has good reason to believe their prescription may have changed, are experiencing symptoms needing investigation or when the client needs an emergency appointment.
· £120 off specs annually – this will be discounted from the cost of your frames only, lenses are not covered under the plan. Excludes reglazes.
· 1/3rd off Contact lenses included in the Bayfields product range.
· 1/3rd off Contact lens Solutions included in the Bayfields product range.
· 10% off Eye drops and cleaning products. Supplements are not included.
· Free delivery of contact lenses up to 12 times a year
· 20% off Sunglasses. However, this excludes anything from our Complete Range.
· Free replacements for broken lenses: Up to 2 free daily or monthly reusable lenses to replace lost or damaged lenses.
· 10 Pairs of free Daily Disposable lenses each year: Only applicable where daily disposable contact lenses are available in a suitable prescription for you.
10) Pay as you go clients will not receive the benefits listed for Belong clients, and will pay the standard charges for their appointments, as per their local branch’s current pricing list. Pay as you go clients will also not receive any of the benefits listed for a Belong Contact Lens Plan client.
11) Records of any benefits redeemed under the Pay as you go and Belong plan will be held on your client record.
12) The subscription is available to those aged 18 yrs and over.
13) Benefits and offers are subject to appointments available in practice.
14) Offers listed cannot be used in conjunction with any other offer or voucher, no alternative or cash exchange is available.
